Neptune Chiropractic and Wellness is a licensed medical examiner listed on the Federal Motor Carrier Safety Administration (FMCSA) National Registry, as mandated by the Department of Transportation (DOT), as well as a health care provider licensed to perform physical exams in the United States, that indicates the results of the components included in School Entry Health Exam form (DH3040-CHP-07/2013), and Florida High School Athletic Association Preparticipation Physical Examination.
DOT Physicals
Florida Department of Transportation requires a physical examination by a licensed medical examiner listed on the Federal Motor Carrier Safety Administration (FMCSA) National Registry. A DOT Physical Exam is valid for up to 24 months. Upon successful completion of the physical exam, Neptune Chiropractic and Wellness will provide one copy of results to the commercial vehicle operator, and complete a medical examiners certificate.
School Physicals
Go Bulldogs! No matter what school you go to, Neptune Chiropractic and Wellness offers Florida Department of Health approved school physicals for children entering the Florida School System, as well as sports physicals. Florida requires all new students grades pre kindergarten through twelfth grade who are enrolling in a Florida school for the first time to submit a school entry health examination. Additionally sports physicals are required by law for all student athletes.
